*This service is for women and covered dependents who are at least 40 years of age and have not had a mammogram in the last 12 months. Screening is free for women 60 years and older with Medicare coverage. Otherwise, cost of mammogram will be billed to your health insurance provider, with co-payment due at time of the visit. Sliding scale financial assistance may be available for those with no insurance coverage whose income is below 200% of the poverty level.
